Care Coordination

Clients work with a care coordinator who advocates for them and helps overcome obstacles they may face in obtaining community services.

Our Care Coordinators can help with:

  • Support Navigation of Medical Care
  • Food Pantry
  • Housing
  • Medication Assistance

Eligibility Requirements for Care Coordination

You must enroll in one of our programs detailed below in order to receive care coordination.

We offer services ranging from STD/STI testing to Trans Gender Non-Conforming (TGNC) care to our clients.

Reproductive Justice (RJ) Program

The RJ program provides reproductive care for birthing people. We provide emergency contraception and family support, such as infant care items, household supplies, and wellness items to those in need.

Food Pantry

The food pantry is open by appointment only to active FAITH and RJ members. Members can call to set up an appointment time at 334-526-2218. For emergency assistance after hours call 334-603-1716.

Housing Support Eligibility

Members can request up to $600 in housing assistance every six months. If you have had an active request for an amount of $600 or above in less than six months from your last request,  you will not be eligible for additional housing  assistance. Only active members (enrolled in FAITH) who meet the eligibility requirements covered in the initial assessment and member handbook will be eligible to receive housing assistance.
